The boss of Heitai no Kuni is the Triple Head, a giant 19th-century Phrenological Head who sits in place at the top of the platform while spitting marbles at Alice; he can also "triplicate", and the point value is 50000.

For those of you who haven't noticed, this world is a paletteswap of the fourth one; so once Alice has dropped onto the platform and the world's theme starts to be heard from the Yamaha YM-2151, start walking forward by pushing the joystick into that "north-east" position as a group of ten Spades come walking towards you while firing marbles at you. Once you've blown a bubble at all of these, to knock them off the edge, you'll have to jump onto a small chequered platform moving left and right, in order to be carried over to the next platform - and once you've jumped onto the next platform, twenty more Spades will come walking towards you while firing marbles at you. Once you have blown a bubble at all of these, to knock them off the edge, you will have to jump onto another small chequered platform moving left and right in order to be carried over to the next platform; once you have jumped onto that next platform, twenty more Spades and eight Dia will come walking towards you, while firing marbles at you. Once you've blown a bubble at all of them to knock them off the edge, you will come to a Box in the centre of the platform - and once you've blown a bubble at it to break it open it will leave a Red Crystal behind. Once you have collected it, you will come to several sections of the platform, that are moving left and right; while you are walking over these, you will come to three more Spades, that are firing marbles at you. Once you have blown a bubble at each of them to knock them off the edge you will come to another Box - and once you have blown a bubble at it to break it open, it will leave a Green Crystal behind. Once you have collected it, you will come to nine Bombs that are bouncing in place; once you've blown a bubble at each of them to make them explode, you'll have to jump onto a third small chequered platform moving left and right in order to be carried over to the next platform. Once you've jumped onto the next platform, twenty more Spades and eight more Dia will come walking towards you while firing marbles at you - and once you've blown a bubble at all of them to knock them off the edge, the platform will narrow into two separate paths as you come up to one of two different third Boxes (both of which will leave an additional Red Crystal behind). However, if you chose the path on the right you'll have the chance to jump onto a fourth small chequered platform moving left and right in order to be carried over to a long platform with two more Boxes on it; once you have jumped onto it and blown a bubble at both of the Boxes to break them open, that fourth one will leave a Mini Watch behind and that fifth one will leave a Purple Crystal behind. Once you have made it back to that main platform, you'll come to several more sections of platform moving left and right; while you're walking over them, you will come to four more Spades, that are firing marbles at you. Once you've blown a bubble at each of them to knock them off the edge, six final Spades will come walking towards you while firing marbles at you - and once you have blown a bubble at each of them to knock them off the edge you will have to jump onto a fifth small chequered platform that is moving left and right in order to be carried over to the next platform (it is small and rectangular), then jump onto a sixth small chequered platform moving back and forth in order to be carried over to the eighth boss platform. Once you have jumped onto the boss platform, the "boss" theme will start to be heard from the Yamaha YM-2151 for an eighth and penultimate time, as you're drawn into a battle with Triple Head; a giant 19th-century Phrenological Head who sits in place at the top of the platform and attacks by spitting marbles at Alice from his mouth (much like a larger version of the Skulls). Once you've blown enough bubbles at him, he will create two doubles of himself in two random positions on the platform - and all three of them will spit marbles at Alice simultaneously. But only one of them is real, and that is the one against whom Alice's bubbles will not dissipate when blown at him; once she has blown enough bubbles at the real Triple Head, both of his two doubles will disappear. The "stage clear" theme will be heard from the Yamaha YM-2151 for an eighth and penultimate time as he explodes and leaves a portal to the final world which Alice will promptly jump into - and you will then receive 10 bonus points for every tenth of a second you have remaining. The ninth and final game world is also a palette-swap of the first one.
