Unlike Fatal Fury Special, which was just an updated version of Fatal Fury 2, Real Bout Fatal Fury Special differs enough from Real Bout Fatal Fury that it is practically a new game in itself. The game takes place after the death of Geese Howard, who returns as the secret final boss "Nightmare Geese". Cheng Sinzan, Tung Fu Rue, Laurence Blood and Wolfgang Krauser all return with the entire cast of RBFF.

Andy, Billy Blue Mary and Tung also have EX versions of themselves with different special moves. Among the new changes from RBFF, is new backgrounds, character sprites, music and new plane switching system.
