The Karate Man 2 stage features a black and white sprite of a karate man who has to punch objects that fly in front of him to the beat of the music. There are four item types: The flower pot, the most common item, the light bulb, the rock, and the bomb. On the left side is a heart meter with five slots, that fills up with one when you hit an item exactly on. light bulbs can be hit at any health, but rocks and bombs can only be hit if you have three hearts or more. If you get a "near miss" on any item (where you hit it and it gets knocked to the floor instead of off-screen) or hit the rock or bomb without three or more hearts, the heart counter drops one. If you miss altogether, it empties. Unlike the first one, the level will change its speed, becoming much faster or much slower to throw you off.

The lyrics are sung by a Japanese woman in English despite being a Japanese video game. While the game is slow down or sped up, the sound of the lyrics are distorted.
