Back 4 Blood is the spiritual successor of the Left 4 Dead series, also created by Turtle Rock Studios. Four players join forces against the Ridden, humans infected by a form of parasitic worm, in order to push back the infection.

While similar to Left 4 Dead in terms of gameplay, the story is not as bleak and hopeless - bringing it closer to the likes of World War Z than 28 Days Later. The Devil Worm is a prehistoric worm found in the Canadian north, brought back to laboratories, and then began infecting the world. The remnants of humanity continue to fight back, with Back 4 Blood's gameplay focused on Fort Hope in America being the center. Also unlike Left 4 Dead, Back 4 Blood features a continuous campaign.

Back 4 Blood features two main modes: the Campaign mode, which has players play through the story - separated into four acts - and Swarm mode, which is the PvP game mode. Swarm Mode has four cleaner players fight against four ridden players in a gradually shrinking arena. After the cleaner team is defeated, they become the ridden team and must defeat the cleaner team. Whichever team survives the longest as the cleaner team wins the round. Each Swarm mode game is played best out of three rounds.

While there is a solo game mode, it is currently not possible to receive any rewards (or achievements) by playing it.
