Akara, High Priestess of the Sisterhood of the Sightless Eye, gives you this quest after introducing herself when you interact with her in the south corner of the Rogue Encampment. Although optional, it's definitely worth completing for the reward.

There is a place of great evil in the wilderness. Kashya's Rogue scouts have informed me that a cave nearby is filled with shadowy creatures and horrors from beyond the grave.

I fear that these creatures are massing for an attack against our encampment. If you are sincere about helping us, find the dark labyrinth and destroy the foul beasts.

May the Great Eye watch over you.

Look for the Den in the wilderness outside the Rogues' Camp.

If you talk to anyone before leaving the camp:

I should add that many Rogue scouts have died in that horrible place. We cannot afford to lose any more. If you choose to enter that Den of Evil, you must do so alone. The demons in that cave have claimed many of my finest archers. I wonder how you will fare! The beasts from the cave have begun to roam throughout the countryside. You'd better be careful out there. One who seeks that cave, seeks death. You're a brave soul! I'd sooner thrust my sacred scepter into the foulest, carbuncular trull than set one boot into that cave.

Blood Moor[edit]

Follow the path away from the bonfire in the middle of the camp to exit it and enter the Blood Moor: this area is only populated by Fallen, Quill Rats and Zombies, with no Champion or Unique monsters in Normal. Continue following the path through the wilderness until it forks. If the first fork takes you to the Den, then you may want to use a Scroll of Town Portal to open a portal outside before retracing your steps and taking the second fork to the entrance to the Cold Plains, which is guarded by the redoubtable Rogue, Flavie:

Halt! You should complete Akara's quest before venturing further. Search for the Den in the wilderness closer to camp. Turn back! I can tell that you need more experience to fight safely in the next wilderness.

She herself is invulnerable and ignored by any monsters, although no experience is gained from any that she kills for you. You can ignore her warnings as the waypoint for the Cold Plains is just inside by the path: once activated, you can use it to return to the camp's waypoint, then use the portal to the south of the central bonfire to return to the Den.

Den of Evil[edit]

If you didn't speak to Akara, this quest is activated upon entering:

Kill all the monsters in the Den.

If you return to camp to talk to anyone before completing this quest:

Your task is not complete until you have killed all the demons in that cave. You'd better come through on this. Your reputation depends on it. You haven't cleared the cave, yet? Do you need anything? One who hesitates... does so with good reason. Demons still befouling that cave, huh? I think you might need a new weapon.

The Den is populated by Zombies, Wendigos and Fallen Shamans accompanied by Fallen, who can be raised after death by their Shamans (so make these your priority). Although you'll encounter your first Super Unique monster at the back of the Den, there are no other Champion or Unique monsters here in any difficulty. Your quest log alerts you when you're close to completing this quest:

Corpsefire[edit]

Super Unique Zombie (Spectral Hit)

Corpsefire will be found at the back of the Den. He and his minions are higher level than other Zombies, giving them greater defense, life, attack rating, damage and experience, but also allowing them to drop higher level items. Corpsefire's Spectral Hit bonus also adds random non-physical damage to his attacks, giving him the ability to poison you, or slow you with cold. When he and every other monster in the Den has been killed, its darkness will be illuminated by streams of light.

Return to Akara for a reward.

If you return to camp via the waypoint just inside the Cold Plains, Flavie's warnings have changed:

Flavie
Beware! Beyond lies mortal danger for the likes of you! Beware! The evil is strong ahead. Take care! The Corrupted Rogues in the wilderness ahead are not to be trifled with.

If you talk to anyone other than Akara upon your return:

Hmm. I'm surprised you survived that test, outlander. Go see Akara. She may reward you. You are truly brave and skillful... Akara was worried about you. ...That which does not kill you makes you stronger. The only good demon is a dead one, I say.

By the way, did you happen to find anything in that cave you'd like to sell?

Talk to Akara, and she rewards you with a skill point and gives you the option to reset stat/skill points.

You have cleansed the Den of Evil. You've earned my trust and may yet restore my faith in humanity.

Your reward is training in the skill of your choice.

In a game created by someone who's already cleansed the Den of Evil, it's renamed the Cave Level 1 (not to be confused with the Cave Level 1 in the Cold Plains, which remains a different area).
