Altered Beast is a linear game. In order to complete it, you must reach the end of every round and defeat the boss in order to advance to the next round. All five rounds must be completed in their order.

To engage the boss of a round, you must reach the god Neff and get him to transform. To make him transform, you must achieve the beast transformation for that round by collecting three power-up orbs from three different silver wolves encountered in the round. If you reach Neff before achieving transformation, Neff will retreat, and you will be forced to continue playing the round, in search of more power-up orbs to complete the transformation. There will always be exactly three silver wolves before you reach Neff for the first time, ensuring that you are not forced to continue playing through the stage unless you miss one or more of the silver wolves.

However, if you reach Neff a third time and still have not transformed, Neff will give up and transform into the boss anyway. Since the bosses are designed to be beaten by the beast you transform into for that round, defeating the boss of the round as a human, no matter how strong, is quite difficult to do.

The amount of bonus points you earn for defeating the boss is also determined by how many times you encounter Neff before he transforms. If you make Neff transform the first time you meet him, you will be awarded 100,000 bonus points. If you beat the boss after encountering Neff twice, you will be awarded 50,000 bonus points. And if you beat the boss after encounter Neff three times, you will only be awarded 20,000 bonus points.

After you defeat each boss, Neff will drain all of the power-up orbs that you collected before warping to a new location. You will automatically jump into the warp behind him and end up in a new location at your original level of power, and you must proceed to collect the power-up orbs all over again.
