From Quake II, PC version, the Rebreather, here seen in the Quake_II/Torture Chambers/Torture Chambers, allows the player to breath underwater for 30 seconds. Without it, you begin to drown after 12 seconds. To stay underwater for 54 seconds, activate a Rebreather 12 seconds after submerging.
