Naruto: Gekitou Ninja Taisen 4 (often Gekitou is romanized as Gekitō) is the fourth installment of the series. It is the second installment of the series released exclusively in Japan. The game covers the events of the anime/manga from the Return of Itachi arc to the Sasuke Retrieval arc. It introduces the ability for multiple characters to fight with each other. The game also removes the shop feature and introduces the mission mode as a method for obtaining unlockables.

Similar to its prequel, Naruto: Gekitō Ninja Taisen 3, it was never released outside of Japan due to the late releases of Clash of Ninja and Clash of Ninja 2.
