The controller maps its buttons to buttons found on a standard Playstation analog controller (buttons are not pressure sensitive as on the Dual Shock II). The following buttons are mapped on the guitar:

The left D-pad button (Playstation-Dpad-Left.png) is an indicator for Guitar Hero to determine if you've plugged in a guitar controller- it must be set to constant "on" for the game to recognize the unit as a guitar. The other buttons (D-pad right, R1, R3, L1, L2, L3, the right stick, and the remaining axes of the left stick) are unused.
