This is one of those situations where the enemy will not do anything until you start firing, and then you can take as much time as you want to make sure you have killed as many of the thugs as you can get without coming out of cover.  
If you still have the RPG, hit the second car back from you and it will blow up some bad guys plus some nearby cars. Hang onto the RPG because another car will pull in from the left in a second and you can take it out too. Switch to an automatic weapon to take out any thugs who are up close. Don't worry about the ones way in the back by the truck.


You may as well start with a well-cooked grenade near the cars up front. Then immediately change to an automatic weapon. These thugs are very aggressive and many of them stand out in the open. If you lock onto one and come up to shoot him, the others will hits you with bullets, so you should use blind aims (pull LT half way and aim with R) in the Cover mode to target thugs who are in the open.  
Once you have killed all the nearby thugs, look to your right. You can run over and climb on top of whatever the thing is next to the wall, then climb up on the wall, which puts you on the roof of a building.


Some will try to charge your position and you have to target them and take them down quickly, so at the start, you should keep the aiming circle on the closest thugs or even just keep it pointed at the bottom of the stairs until you have gotten those 4 or 5 who are going to charge you.
From there, you can shoot some more bad guys, if you wish, but keep an eye out for the truck at the far end to make sure it doesn't leave without you. Eventually you will want to cross the roof and jump down to the street, then continue left to the alley which the truck is going to come down.


After you get the thugs who were in the open, you will have to deal with those behind cover. You can get most of them with blind aims. One or two may be using blind aims on you, in which case the easiest way to get them is to lob grenades at them. Be sure to aim the target circle high enough to get the grenade over cars.
There is no need to go down the alley and get shot, just wait for the truck to come out and run/jump for the marker trailing behind it. The rest is pretty easy, following the instructions on the screen.  
 
Once you have finished off all the nearby thugs, pull out your sniper rifle and zoom in on the area to the right of the truck. You should see 3 or 4 thugs and be able to pick them off. Once you get them, you probably won't be able to see any of the remaining thugs near the truck, but the truck is going to pull out soom and you must run after it.
 
Thugs will be shooting at you as you run after the truck, which is why you need to have preserved your armor/health. The truck is slow and at a sprint, you should catch up to it. The rest is pretty easy, following the instructions on the screen.  


The key to capturing the truck once you are on top is to press L back (towards you) to grab hold during turns, then press L forward to get to the cab of the truck while going down straight streets.

Once you get the truck, you will be told to drive it to an old mansion. After you do so and are reaady to leave, you'll see a motocycle nearby, but don't take it. Turn around and go to the rear of the garage and you will find a coveted Sultan sports car in the weeds; take it instead.
