The primary and secondary gadgets refer to your given class' special devices. For example, the medic's ability to drop health packs and resurrect teammates (or kill enemies) with the defibrillator are referred to as the secondary and primary gadget respectively.

In a Vehicle

Secondary fire usually refers to using whatever vehicle specialization the player has equipped. For example, equipping the smoke countermeasures package and pressing the button will trigger the smoke screen.
