You play as one of the Autobot Headmaster through four planets, fighting the Decepticons. Unlike it predecessor, [[Transformers: Convoy no Nazo]], players cannot switch between robot and vehicle mode, as they are predetermined by level. Players also cannot choose which Autobot to play. Players must collect them throughout the game, as they will be flown in by helicopter and will replace that Autobot when they die. Due to Headmasters being for the FDS, the game came with a save game feature similar to those found in early {{c|The Legend of Zelda}} and {{c|Metroid}} games.

The title of this game can be easily skewed. First, understand that the Japanese equivalent of ''Transformers'' is "トランスフォーマー", which is pronounced "toransufoomaa" and sounds like the word is singular. Because the name is then followed by "ザ" (Za), some readers confuse the Transformers name to include this character (thus making it pronounced "toransufoomaaza" and sound plural). As such, the title can be misrepresented as '''Transformers: Headmasters'''. Alternatively, it could be translated as '''The Transformers: Headmasters'''. The final mistake some readers make is that "ヘッドマースターズ" (Headmasters) includes the "ズ" (Zu). By excluding the "ザ" from "トランスフォーマー", it is also possible to mistakenly exclude the "Zu" from Headmasters. If this happens, the translation may come out to '''The Transformers: The Headmasters''' (or something more grammatically incorrect, such as ''Transformers: The Headmasters The''). Luckily, the box art visually separates the three words.
