Prefixes appear at the beginning of an item's name. Note that there are two types, normal prefixes and upper level prefixes that only apply to Purple and Orange items. These upper level prefixes have special names that make these rarer items seem more unique. Also note that these upper level items can have many prefixes - however the maximum number of effects allowable on any item is six.
The easiest way to find the prefix you are looking for is to press Ctrl + F and search for it. Note that some prefixes have different qualities for either armor or weapons, so double check that you are looking at the correct one.
No one knows what the correlation between these prefixes and the modifiers on the item are. There's always the chance that they are just for aesthetics.
