Then Tommy has to take him to the hospital, so follow the marker there. With Tommy inhaling the boomshine, it's hard to drive and the screen moves, so drive carefully and watch the time limit. Phil is hallucinating, and by the time Tommy reaches the hospital Phil protests "Too many cops and Viet Cong" and Tommy must take him to his primary care physician.
