The game follows Sonic the Hedgehog as he explores the Starfall Islands on a quest to save his friends and uncover the islands's mysteries, while contending with the monsters and Titans that inhabit the islands, as well as the mysterious girl Sage.
 
In a series first, the game incorporates open-world elements, as Sonic is free to roam the islands, collect items, fight enemies, and even fish at his leisure. Combat has also been expanded, giving Sonic access to a wide variety of attacks that are gradually unlocked via a skill tree system.
